Closing date for registration: 3 December 2018

Applications are on “first-come-first-served” basis. 
Successful applicants will be informed by 10 December 2018. Course fees must be received by 4 January 2019. Sponsored individuals must get their HR consent and the fees would be billed directly to the organisation. 

Course Fees

Singapore Dollars 1,337.50 (Singapore Dollars One Thousand Three Hundred and Thirty-Seven and Cents Fifty only) per participant per course. Applicable for all courses except for Principles of Workplace Health.
Singapore Dollars 4,173 (Singapore Dollars Four Thousand One Hundred and Seventy-Three only) per participant ONLY for the course Principles of Workplace Health.

The fees quoted are inclusive of the prevailing Goods and Services Tax.

Please acknowledge that the payment is due by 4 January 2019.

Cancellation Policies

A cancellation charge of 50% of course fees will be levied if any cancellation notice is received before the stipulated date for the respective course.

No refunds will be given for any cancellation notice received from the stipulated date for the respective courses onwards but replacement with another participant is permissible.

The organizer reserves the right to cancel the course and fully refund the fees in the event of unforeseen circumstances.

Any cancellation must be conveyed to the course administrator in writing via email. A cancellation charge of 50% of fee will be levied if the cancellation is received BEFORE 4 January 2019.

NO REFUNDS WILL BE GIVEN FROM 4 JANUARY 2019 ONWARDS but replacement with another name is permissible.

Cheques/ Bank drafts (drawn on a bank in Singapore in Singapore dollars) to be made payable to the "National University of Singapore". All cheques have to be valid for at least 3 months upon receipt by the School. All bank charges must be borne by the payer
