Registration FormAsia Pacific Student Services Association (APSSA) Roundtable 2019 Singapore Theme: Mental Wellness in Higher Education Thursday 12 December & Friday 13 December 2019 Jointly organised by National University of Singapore (NUS) Nanyang Technological University, Singapore (NTU)Singapore Management University (SMU)Registration Fee: Waived (supported by NUS, NTU and SMU)Notice: By participating in this event, you hereby fully consent to the organisers collecting, using and/or disclosing your personal data in any form and to disclose the same to third parties (including any third party located outside of Singapore) in compliance with the Singapore PDPA 2012 and all subsidiary legislation for the purpose of APSSA Roundtable 2019 Singapore (the “Event”), subsequent registration to and all other actions necessary in relation to the Event. Participants should be aware that the organisers has appointed authorised personnel (who will be carrying identification) to take photographs and videos for and on behalf of the organisers during the Event. The organisers may use photographs and videos taken by such authorised personnel for APSSA marketing and publicity purposes in print, electronic and social media. The organisers shall not be responsible for photographs and/or videos taken by unauthorised persons during the Event.
Agreement FormFor Participation in APSSA Roundtable 2019 Singapore and Acknowledgement to Release all Claims and Agreement to Indemnify NUS. Please read the following carefully and submit the form.
1. Participation a. I agree to attend all of Asia Pacific StudentServices Association (APSSA) Roundtable 2019 and activities, leaving and returning together with my university colleagues; b. I agree to submit personal information requested(i.e. registration form), in their entirety, at the time they are requested; c. I agree to communicate, as well as be available to organising staff as issues arise about participant status, attendance, and project information; d. I agree to fully participate in all data collection and program evaluation efforts by submitting all requested information and materials by scheduled due dates. e. I will give my solemn promise to maintain these rules during APSSA Roundtable 2019 programme, December 12th and13th, 2019, and conduct myself with responsibilities as one of the representatives of my university. 2. Support of the Programme a. I pledge to observe the regulation and follow decisions made by Asia Pacific Student Services Association (APSSA) Roundtable2019 b. Compliance with Rules - I agree that I will abide by all programme policies, regulations, and procedures and by all local, state and federal laws. If I fail to abide by these rules and laws, that may be a basis for denying or ending my participation in this activity. c. I agree that the organising committee may photograph, videotape or record me in connection with this activity. I agree that the organising committee will be the owner of all images and recordings and own all copyright in the images and recordings. The organising committee may use these images and recordings for advertising or other media releases. d. For security reasons and to comply with local authority restrictions I am not allowed to invite or allow any person whois not a current delegate or staff member to enter my accommodation without the express consent of the organising committee. 3. Terms of Release from the programme - I acknowledge that I will be released from the Programme for cause if one or more of the following circumstances exist: a. The Participating in any of the prohibited activities. b. The registration form and process are consistently not completed, in their entirety, or submitted by their respective due dates. c. Consistent lack of performance by the participant in accomplishing programme objectives. d. Inability to provide required financial match to receive Programme services. e. Violation of any Federal, State or Local laws. 4. Financial Responsibility a. I understand that I am responsible for obtaining insurance and for any expenses that arise out of medical care. b. I pledge to provide compensation for damage I cause to the organising committee hereto through willful misconduct or negligence. c. I agree that payments referred to herein shall not be refundable under circumstances, including but not limited to the termination of this Agreement for whatever reason. 5. Inherent Risks a. I understand and acknowledge that there are known, unknown, and unanticipated risks and dangers that are qualities of this activity that cannot be eliminated. These are often called “inherent risks” and will be referred to this way in this agreement. b. I agree that neither the Asia PacificStudent Services Association (APSSA) Roundtable 2019 organising committee are liable in any way for damages by Force Majeure such as natural calamities and internal insurrection.
